Our doctors suggest that stretches and/or exercises at home be part of the basic rehabilitation program. Home exercises such as lying in bed and then rolling from side to side with the knees flexed and then extended, rotating from side to side standing, and lying flat on the back and moving the legs as if pedalling a bicycle may reduce symptoms. Taking warm baths, in which the full body is immersed, may also be helpful. Cold packs may help reduce inflammation. Massage may reduce muscle tension, and yoga may help.
Piriformis syndrome is caused by spasm of the piriformis muscle, which compresses or irritates the sciatic nerve. The piriformis muscle runs through each buttock diagonally and is important for rotation of the hips and maintaining stability during walking. Injury to the piriformis itself as well as poor biomechanics and/or overuse can result in piriformis syndrome. The symptoms of piriformis syndrome include a dull ache in the buttock and often pain travelling down the back of the leg, it may be worse after prolonged sitting or running and may be associated with stiffness of the hip.
Piriformis syndrome may be diagnosed using a combination of physical examination, medical history and in some cases, imaging to rule out other causes of pain in the same area. Pain with certain movements of the hip and localised tenderness of the buttock is characteristic of the condition.
There are a number of effective physiotherapy-based exercises, stretches and massage for Piriformis syndrome, but the mainstay of management is specific regular stretching of the muscles, incorporated into a more generalised regular exercise and fitness regime. Posture and biomechanics need to be optimised to prevent recurrence of the condition.

In severe cases, powerful muscle relaxants may be injected directly into the Piriformis muscle in order to stop the spasm.
